Carer's emergency card

It is free to apply for a carer’s emergency card.

To apply, you need to fill in this form and send it to Ealing Council.

You can send it by:

After you apply:

You will get a card and a small key fob - you should carry them with you at all times.

Each year, the council will contact you to check your details are still correct.

You can update your emergency plan at any time by contacting the adult social care team.
