Applying for a carer's emergency card

It is free to apply for a carer's emergency card.

To apply you need to complete the registration form and return it to Ealing Council by:

Following your application you will be sent a credit card sized card and key fob to carry with you at all times.

You will be contacted yearly to review the details you have provided, to keep them up to date.

You can update your emergency support plan at anytime by contacting the adults contact team using the details above.

You can get a paper copy of the form, and assistance with filling in the form, from the Ealing Carers’ service at
