Apply for a fitness and small group activities permit

Use this form to ask for permission to hold an activity in a park.

Before you start

Send an initial enquiry email to ealing@theeventumbrella.com and wait until we tell you, to make your application.

Read the supporting documents, including the terms and conditions and fees and charges.

Do not use this form to apply for:

  • an event in a park with 50 people or more, on site infrastructure or music, catering or other coordinated activities
  • street parties
  • hiring a sports pitch, use the Playfinder website

You will need 

  • applicant details, the applicant is the individual or organisation as named on the application form, who enters into formal agreement with the council and who is responsible for the activity
  • a description of the activity
  • the activity name
  • the sites you plan to use
  • dates and times it will run, we need specific dates for irregular or ad-hoc sessions specific dates
  • names, experience, and qualifications held by those delivering the activity
  • the maximum number of participants you would allow at each session. Permit fees are based on maximum attendance, at the time of application, not actual attendance on the day
  • activity permit holders are not permitted to bring infrastructure to site without written consent from the council beforehand. If you would like to request an exception to the terms and conditions, for example tents, gazebos, music, lights, or toilets, you will need to give details and include the risks in your activity risk assessment
  • a site map, which names the site you wish to use and defines the boundaries of your proposed activities. not just a location point
  • emergency arrangements, which outlines the cluster points, access points, named responsible person, contact details, first aid arrangements and nearest hospital
  • risk assessment, for your proposed activity
  • safeguarding of minors and vulnerable persons, for example if your activity involves young people, DBS certification is needed under current guidelines. We will need you to certify that DBS certification is current and valid for all providers. We will ask for a copy of your child protection information and details of specific arrangements
  • insurance, evidence of your public liability insurance, £5 million cover

Uploading supporting documents 

You will be asked to upload some documents.

  • accepted file types include: Word (doc, docx), Acrobat (pdf), OpenDocument text (odt) Plain text (txt, rtf)
  • we also accept photos taken on phones and tablets and the following image file types: bmp, gif, jpeg, png  
  • Zip and Excel (xls) files cannot be uploaded

All applications are subject to final approval by the events service. 

You have the option to be added to the list of registered fitness providers on our website

This form should take around 45 minutes to complete.
