What is a social care assessment

Eligibility criteria

When carrying out an initial conversation or social care assessment, the conversation will aim to understand how well you are able to meet a nationally defined set of criteria; and the impact on your wellbeing where you are not able to meet them fully.

The nationally defined outcomes measure you ability to achieve the following, and the impact it is having on your wellbeing:

  • Having a relationship with friends and family
  • Using community facilities and services
  • Engaging in education, work, training, or volunteering
  • Caring responsibility for a child
  • Being able to use your home safely
  • Maintaining a habitable home environment
  • Managing and maintaining your nutrition
  • Maintaining personal hygiene
  • Managing toileting needs
  • Being appropriately clothed

Eligibility outcomes for adults with care and support needs under the Care Act 2014
