This form should only be completed by residents who are facing a financial crisis. Check if you are eligible before making an application.
In order for us to be able to consider your application you will need to tell us in detail about:
- the crisis you are experiencing
- why you are not able to pay for basic living expenses

We can only make payments if we have all the evidence required to support the application. At the end of the form you will be asked to upload the following documents:
- proof of identity - passport, driving licence, birth certificate etc
- proof of address - council tax bill, utility bill, benefit entitlement letter, tenancy agreement etc
- proof of capital - up to date statement or mini statement for all accounts held
- proof of earnings - latest payslip or your most recent Universal Credit statement
We are not able to assess applications that do not include the evidence required. If you need help to upload documents you can visit one of our community hub locations
Uploading supporting documents
- we will accept clear photographs of documents
- other accepted file types: Word (doc, docx), acrobat (pdf), excel (xls, xlsx), plain text (txt), powerpoint presentation (ppt, pptx), image (jpg, jpeg), rich text format (rtf)
- zip files cannot be uploaded
- do not upload files larger than 10MB
We aim to process applications as quickly as possible, but timeframes may vary depending on the volume of requests. You can help us make quicker decisions by providing all supporting documents together with your application.
If your application is successful, your award will be paid via BACS transfer. Any award made must only be used to pay for items you have listed on your application.
