How to apply

Older persons applications

Residents aged 60 years or over need to apply online at 

Alternatively, you can get a paper application form from Perceval House or local libraries. You may also telephone 020 8825 8161 to request a form by post.

Disabled persons applications 

For blind or disabled persons Freedom Pass applications, please print out and complete the Freedom Pass application form. Once you have filled this in, attach the required documents and photograph and send to: Ealing FP, PO Box 68276, London W5 9LQ

If you are returning the form in person to Perceval House, please place all documents in a sealed envelope and use the 'drop box' provided. This may delay your application as we will need to forward it to the PO Box address for processing.

Applications take approximately 21 days to process from the date all documents are provided. If successful the pass will be posted to your home address. Your application may take longer to process if you need to attend an occupational therapist assessment appointment to decide on eligibility.
