You can refer an adult for a social care assessment if they:
- have trouble with daily tasks, and support from the community or high street equipment has not helped
- want to change something in their life, but cannot do it alone or with help from carers, family, or friends
Before you start
Check if you or the person can find the right help online first:
- a range of community support options in Ealing is listed on CarePlace
- many items of equipment can be bought from local shops, such as pharmacies, or online
To complete this form, you must:
- be making the referral as a professional
- make sure the person knows you are making this referral
- check the person is over 18 years old
- make sure the person lives in Ealing borough
If you cannot get the person’s consent, explain why the referral is still needed.
Urgent referrals
Urgent referrals can also be made by contacting the Advice and Referral Centre on 020 8825 8000.
Telephone referrals should be made:
- if you have a safeguarding concern or want to report abuse
- if it is urgent and support is needed within the next 24 hours
You will need
- person’s name and contact details
- person’s date of birth
- NHS number
- GP surgery name and address (if known)
- full details of the person's current situation
- information about support available
- options you have already explored
- medical summary (if you are a GP)
Sending documents
You may be asked to upload certain documents, like a medical summary if you have one.
- we accept Word (doc, docx), Acrobat (pdf), OpenDocument text (odt), and plain text files (txt, rtf)
- photos taken on a phone or tablet are fine, and we accept bmp, gif, jpeg, and png image files (please do not send any images of people)
- we cannot accept Zip or Excel (xls) files
What happens next
We aim to look at all referrals within 3 working days and may contact you for more information if we need to.
If we have all the information we need, your referral will be processed, and we will either contact the adult to arrange an assessment or we will help them with information, advice and referrals to other services.
