Identity |
Two items of identity are required for the tenant and their partner (if they have one). e.g. birth certificate, marriage certificate, driving licence, passport, bank statements, utility bills for the current address. |

National Insurance Number |
One item is required for both the tenant and partner (if they have one) to prove their national Insurance Number e.g. payslips, National Insurance Card, bank statements showing direct payments of welfare benefits, letters from the Tax office or DWP. |

|
Capital, savings and investments |
Last two months' bank statements (even if overdrawn) for each account, certificate for Premium Bonds, National Savings Certificates, ISA’s, stocks shares and unit trusts. |

| Earnings | Last consecutive five weekly payslips, two monthly payslips or three fortnightly payslips. If you or your partner is self-employed, we need to see your accounts for the last financial year or, if you have been trading under six months, a summary of your trading records so far. | |
| Child benefit | Letter from the child benefit unit or bank statements showing payment of child benefit. | |
| Other income | Pension slips for private pensions, court orders for payments received through court, letter from ex-partner for voluntary maintenance payments. | |
| Tax Credit | Most recent tax credit notification letter from the Inland Revenue. | |
| Savings Pension Credit | Most recent Savings Credit award notice from The Pension Service. | |

Child benefit |
Letter from the child benefit unit or bank statements showing payment of child benefit |

Receipts or agreements from the registered childcare provider detailing the amount paid out. |

Disability Living Allowance |

Non dependant information |
Proof of gross income for each non-dependant. |

| Proof of rent | Tenancy agreement, current rent book, rent receipts, or a letter from the landlord. | |

Payment direct to landlord |
The Housing Benefit Regulations are due to be altered next year and will only allow benefit to be paid direct to the tenant unless we consider them to be vulnerable. We recommend that payments are made direct into your tenant’s bank account. |

Sharing information |
If you want to check the progress of your tenant’s claim has your tenant signed the 'Sharing information with your landlord' section of the housing benefit form. |

Date moved in |
The tenant must state the date that they moved into the property. If they have not moved in yet do they know that they have to confirm in writing the date that they actually move in? |

About the property |
The tenant must state the type of building they live in. If they live in a flat, bedsit or rooms they should identify: - the floor they live on
