If your child is due to start secondary school in September 2009 and was born between 1 September 1997 and 31 August 1998 applications online should have been made between 1 September 2008 and 24 October 2008 for any London state school.

When you have submitted your application you will be given a unique numbered password. It is essential that you keep this information to enable you to view the outcome of your application.
If you have submitted your online application and you do not receive your unique reference number on screen, the application has not been successfully submitted.
Important: If you choose to apply online you must not under any circumstances complete a paper application as well.

(i) Documentary evidence as proof of child’s date of birth
and permanent home address
You must send us, with a printout of your online
application, the required
documentary evidence as proof of your child’s
date of birth and permanent home address. It is important that you
write at the top of all documentary evidence you submit, the unique
reference number you are given when you apply online.
(ii) Professional evidence to support claim for exceptional
medical or social circumstances
If you are asking for exceptional medical or social
circumstances to be considered in respect of your application, you
must, in addition to the documents required in point (i) above,
send us a letter or report from your child’s consultant (if you are
claiming exceptional medical circumstances) or from a relevant
professional (if you are claiming exceptional social
circumstances). It is important that you write at the top of all
letters or reports you send to us, the unique reference number you
are given when you apply online and ensure that your child’s name,
date of birth and address are included in all the documents.
